Clinical signs:

• Signs and symptoms of CHF at rest
• Optimal dose of diuretic and vasodilator therapy
• Ejection fraction of 20% or less
• Cardiac symptoms:
  • Arrhythmias resistant to therapy
  • History of cardiac arrest
  • History of syncope
  • Cardiogenic brain embolism
